• Covers the entire hospital, rather than a specific unit.

  • 10 Case Managers
  • 3 Social Workers
  • 2 CM Assistants
  • 1 Manager
  • 1 Director
• Typical caseload: 20–25 patients per RN; approximately 50 patients per Social Worker
• EMR: Meditech / Meditech Expanse (transition noted for 8/13)
• Equipment: Computer, phone, etc.

Required Experience

• RN license required
• Minimum 2 years of hospital Case Management experience
• No specific certifications required.
• Candidates without the required hospital CM experience will not be considered.
