• Provide skilled physical therapy services to patients in an SNF setting.
• Perform comprehensive PT evaluations and develop individualized treatment plans.
• Provide therapeutic interventions to improve strength, mobility, balance, gait, and functional independence.
• Monitor patient progress and modify treatment plans as needed.
• Educate patients and families on exercises, safety, mobility, and discharge planning.
• Collaborate with nursing staff, physicians, and the interdisciplinary rehabilitation team.
• Maintain accurate and timely clinical documentation.

Requirements

• Active Washington Physical Therapist license or eligibility to obtain.
• Recent PT experience preferred.
• SNF experience preferred.
• Current BLS certification.
• Strong clinical reasoning, communication, and patient care skills.
