Required Qualifications

Minimum 2 years of recent acute medical Care Manager experience performing care coordination
Recent acute care hospital experience is required
Previous travel nursing experience is required
Strong experience with care coordination and discharge planning
Experience assessing patient needs and developing plans of care
Ability to independently arrange DME, SNF placement, and ambulance transportation
Epic experience is required
