RN Case Manager

💰 Pay: $74/hr W2 | $2,960/week for 40 hrs

📍 Location: Mission Viejo, CA

📅 Start: 09/21/2026

📆 Contract: 13 Weeks

☀️ Shift: Days | 5x8 | 8:00 AM – 4:30 PM

⏰ Hours: 40 Hours/Week

Requirements

Active California RN License
Minimum 2 years of acute-care RN Case Manager/Care Manager experience
Recent acute medical Case Management/Care Coordination experience within the last 3 months – HARD STOP
Previous travel experience – HARD STOP
Epic experience – HARD STOP
Strong Care Coordination & Discharge Planning experience
Must be able to independently arrange DME, SNF placement & ambulance transportation
Experience with HMOs/IPAs preferred
Adult & geriatric patient experience

Responsibilities

Patient assessment and care coordination
Discharge planning and transition of care
Develop and coordinate plans of care
Coordinate DME, SNF placement and transportation
Participate in interdisciplinary and family meetings
Address complex psychosocial and discharge barriers
Maintain accurate Epic documentation

Important: This is a Case Management/Care Coordination role — Case Managers do NOT perform Utilization Review.
