Location: Oak Brook Illinois 

Duration: 13 Weeks

Shift: 8hr Day Shift (40)

Shift Information: Monday – Friday, 0800-1630

Clinical Interviewing- Yes, manager approval to offer

On Call, Call Back Requirements: None

Weekend Rotation: Every 5th Weekend

Holiday Requirements: 50% or 2 holidays in contract possible

Floating Requirements: Will be assigned to a specific unit, but could be float to other inpatient units

About Facility:

• Beds: 205
• Level II NICU
• No trauma designation

Unit Information:

• Beds: 205 is hospital, unit sizes vary
• Census: Average patients/day:

Minimum Years of Experience required: 3 years of acute care case management experience

• Will you accept a first-time traveler: Yes

Patient Types/Common Diagnoses:

• This is an underserved population, mostly adults and geriatrics. Will see complex cases like unhoused and low-income patients
• Could be assigned to PCU, MedSurg, LDRP; will likely not be assigned ICU but possible
• Will be assigned to a specific unit and could float to cover other units

Case Load:

• Typically, around 14-20 patients

Required License, National Certification, Certs:

• No specific certifications required. Will accept ASN or higher

Skills required:

• Working with multidisciplinary team and physicians (comfortable asking questions), navigating various insurance plans, will assist with facility initiatives
• Soft skills: Good assessment skills a must, comfortable finding resources for patients, good critical thinking skills, good interpersonal skills. MUST be a team player
• Will be instructed on programs and resources available once here.

• Leadership
• Other CMs
• CM assistant
• Hospitalists 24/7

Technology/Equipment:

• EMR: EPIC
• Careport for referrals

Floating: If yes, where: 

• Will be assigned to a specific unit, but could be float to other inpatient units

Orientation (facility and unit) :

• NEO
• 3 shifts of orientation in department; possible for 1-2 additional shift if needed

Scheduling:

• Weekend rotation: Every 5 th weekend
• On call: No
• Holiday Expectations: 50% or 2 holidays in contract possible
• Shift times: Monday – Friday, 0800-1630 (may occasionally have a few days staying late), unless weekend on. If working weekend, will have off a Monday and Friday after that
• Schedule cycle: Will have a set schedule with weekend rotation

• Scrub Color: Eggplant colored scrubs or business casual; white lab coat required
