🏥 About the Assignment

This position is primarily based within the Skilled Nursing Facility (SNF)/Extended Care Center (ECC) but may flex into the hospital's inpatient rehabilitation department based on patient needs.

You'll work with a diverse patient population, including:

• Short-term rehabilitation patients
• Long-term care residents
• Post-acute recovery patients
• Inpatient rehabilitation patients

💼 What You'll Be Doing

As a Physical Therapist, you will:

✅ Evaluate and treat rehabilitation patients in the SNF setting

✅ Develop individualized treatment plans

✅ Provide therapeutic interventions to long-term care residents

✅ Collaborate closely with Occupational Therapy and interdisciplinary team members

✅ Participate in discharge planning and patient care recommendations

✅ Attend weekly patient care conferences

✅ Support inpatient rehabilitation services as needed

✅ Help patients improve mobility, strength, function, and independence
