Mental Health & Psychiatric Registered Nurse (RN)

Location: Bronx, NY
Job Type: Contract
Contract Length: 13 Weeks, with possibility of extension
Schedule: Monday–Friday | 10:00 AM–6:00 PM or 11:00 AM–7:00 PM
Pay Rate: $43–$48/hour
Pay offered is based on experience, expertise, credentialing, and education.
 

Position Overview

Pride Health is hiring a Mental Health & Psychiatric Registered Nurse (RN) to support a supportive housing program in the Bronx, NY.

This position is ideal for an experienced RN with strong nursing case management and care coordination skills and experience working with individuals with mental health and/or psychiatric needs.

The RN will support residents' overall health and wellness by coordinating medical care, conducting wellness checks, assisting with follow-up care, providing routine nursing services, and maintaining accurate documentation.

This position does not involve medication administration or medication management.
 

Key Responsibilities

  • Coordinate and assist residents with medical appointments, referrals, and follow-up care.
  • Provide routine nursing support, including wound care as needed.
  • Conduct wellness assessments and routine health checks.
  • Follow up with residents after hospital visits and assist with care transitions.
  • Monitor residents' health needs and coordinate appropriate services.
  • Maintain accurate and timely documentation of resident interactions, medical follow-ups, assessments, and services provided.
  • Utilize the AWARDS electronic health record (EHR) system for documentation and case management.
  • Collaborate with program leadership, healthcare providers, and community-based service providers.
  • Provide nursing case management and care coordination to support resident health and stability.
  • Support individuals with mental health, psychiatric, and other healthcare-related needs.

Program Setting

The RN will work within a supportive housing program serving approximately 86 residents. Residents live independently in their own apartments and are generally able-bodied.

The program serves individuals who may have mental health diagnoses, behavioral health needs, or other barriers, but the population does not require direct support professional (DSP) services.
 

Qualifications

  • Active New York State Registered Nurse (RN) License and Registration.
  • BLS/CPR Certification through the American Heart Association (AHA).
  • Professional Malpractice Insurance.
  • Associate Degree in Nursing (ADN) or higher.
  • Experience working with mental health and/or psychiatric populations.
  • Strong nursing case management and care coordination experience.
  • Excellent communication, documentation, and organizational skills.
  • Ability to work independently while collaborating effectively with multidisciplinary teams.
  • Bilingual candidates are preferred.
