Job Details – RN Hospice

• Position: RN Hospice
• Unit: Hospice – Skilled Nursing
   
• Start Date: ASAP
• Shift: Day Shift, 
   
• Contract Status: Accepting Applications
• Mileage: Mileage is paid in addition to the hourly rate
• Travel Requirement: Traveler must have Hospice experience
• Caseload: Typically 12–18 patients

Required Qualifications

• Active RN license in the state of practice – Required
• CPR or BLS certification from an approved vendor – Required
• Valid Driver’s License – Required
• Hospice nursing experience – Required for this position
• BSN – Preferred
• 5 years of hospice nursing experience – Preferred

Main Responsibilities

• Manage an assigned hospice patient caseload.
• Conduct initial and ongoing patient assessments.
• Develop and update individualized care plans.
• Provide skilled nursing care, including medication management, pain/symptom control, and wound care.
• Educate patients and families about medications, symptoms, and hospice philosophy.
• Coordinate care with physicians and the interdisciplinary team.
• Maintain accurate EMR documentation.
• Participate in interdisciplinary team meetings.
• Follow Medicare, Medicaid, and state/federal hospice regulations.
• Participate in the on-call rotation and respond to after-hours patient needs.
