POSITION SUMMARY

The Social Worker is responsible for assessing residents' psychosocial needs, coordinating services, facilitating care planning, and providing emotional support to residents and families throughout their stay.

The Social Worker works collaboratively with the interdisciplinary team to promote resident well-being, support successful transitions of care, and ensure compliance with applicable regulatory requirements.

JOB REQUIREMENTS

Experience & Skills

• Minimum of 2 years of related experience required.
• At least 1 year of experience in a healthcare setting, such as a hospital, clinic, rehabilitation center, or similar environment.
• Skilled nursing experience is required.
• Excellent problem-solving skills.
• Ability to utilize a customer-focused approach to problem solving and goal setting.
• Excellent communication and listening skills.
• Enthusiastic, resourceful, and organized.
• Strong personal awareness of nonverbal communication.
• Computer proficiency, including Microsoft Office skills.

Education

• Bachelor's Degree in Social Work, Psychology, Gerontology, or Sociology from an accredited institution is required.

RESPONSIBILITIES

• Assess the psychosocial needs of residents.
• Coordinate appropriate services based on resident needs.
• Facilitate individualized care planning.
• Provide emotional support to residents and their families throughout their stay.
• Collaborate with the interdisciplinary team to promote resident well-being.
• Assist with successful transitions of care.
• Support communication and coordination between residents, families, and healthcare team members.
• Promote a customer-focused approach to problem solving and goal setting.
• Ensure compliance with applicable regulatory requirements.
• Maintain appropriate documentation and utilize computer systems, including Microsoft Office, as required.

CANDIDATE PROFILE
