Overview
We are seeking a compassionate and resident-focused Social Worker to join a premier Skilled Nursing Facility (SNF) in Naples, Florida. This role is responsible for supporting residents and families, coordinating services, facilitating care planning, and helping ensure the highest quality of care and quality of life.
The ideal candidate is passionate about resident advocacy, enjoys working as part of an interdisciplinary team, and has prior experience in a skilled nursing or healthcare setting.

Key Responsibilities
- Assess residents' psychosocial needs
- Provide emotional support and counseling to residents and families
- Coordinate social services and community resources
- Participate in interdisciplinary care planning meetings
- Assist with transitions of care and discharge planning
- Advocate for residents' rights and well-being
- Ensure compliance with regulatory and documentation requirements
- Collaborate with nursing, therapy, and medical staff to support resident outcomes
Requirements
- Bachelor's Degree in:
- Social Work
- Psychology
- Gerontology
- Sociology
- Minimum 2 years of related experience
- At least 1 year of healthcare experience in a:
- Skilled Nursing Facility
- Hospital
- Rehabilitation Center
- Clinic or similar healthcare setting
- Strong communication and interpersonal skills
- Excellent problem-solving and organizational abilities
- Proficiency with Microsoft Office
Preferred Qualifications
- Skilled Nursing Facility (SNF) experience
- Long-term care experience
- Experience working with geriatric populations
- Knowledge of care planning and discharge planning processes
