Description
Unit: ED Care Manager
Unit Information:
Beds: 205 is hospital, unit sizes vary
3 years of acute care case management experience
Will you accept a first-time traveler? Yes
Required License, National Certification, Certs: BLS
Will accept ASN or higher
Technology/Equipment:
EMR: EPIC
Careport for referrals
Patient Types/Common Diagnoses:
This is an underserved population, mostly adults and geriatrics. Will see complex cases like unhoused and low-income patients
Could be assigned to PCU, MedSurg, LDRP; will likely not be assigned ICU but possible
Will be assigned to a specific unit and could float to cover other units
Case Load: Typically, around 14-20 patients
Skills Required:
Working with multidisciplinary team and physicians (comfortable asking questions), navigating various insurance plans, will assist with facility initiatives
Soft skills: Good assessment skills a must, comfortable finding resources for patients, good critical thinking skills, good interpersonal skills. MUST be a team player
Will be instructed on programs and resources available once here.
Scheduling:
Weekend rotation: Every 5th weekend
On call? No
Holiday Expectations: 50% or 2 holidays in contract possible
Shift times: Monday – Friday, 0800-1630 (may occasionally have a few days staying late), unless weekend on. If working weekend, will have off a Monday and Friday after that
Schedule cycle: Will have a set schedule with weekend rotation
Scrub Color: Eggplant colored scrubs or business casual; white lab coat required
