Description

Pride Health is actively hiring a Registered Nurse (RN) to join our client's Home Health Team.

 

Pay Rate: Up to $55.00/hour (W-2)
Weekly Hour Guarantee: 32 hours/week
Schedule: Monday–Friday, 8:00 AM–4:30 PM
On-Call/Weekend Requirement: Yes
 

Mileage Reimbursement: Yes – mileage between patient visits, from the first patient through the last patient, is reimbursed.

Work Location: Needham Heights, MA 02494

 

Coverage Area: Boston and surrounding regions, including but not limited to: South Boston, Dorchester, Back Bay, Roxbury, Beacon Hill, South End.

 

Position Overview:

  • We are seeking a Registered Nurse (RN) to provide high-quality, patient-centered care in a Home Health setting.
  • The RN will independently manage patient care, develop and implement individualized Plans of Care, complete OASIS assessments, coordinate with physicians and interdisciplinary team members, and ensure accurate and timely clinical documentation.

Key Responsibilities:

  • Initiate, develop, implement, and revise individualized Plans of Care (POC) in collaboration with physicians and other healthcare professionals involved in patient care.
  • Provide high-quality skilled nursing services within the RN's scope of practice and established infection-control standards.
  • Deliver care in accordance with the patient's Plan of Care and coordinate services with the interdisciplinary healthcare team.
  • Conduct comprehensive patient assessments, including OASIS assessments and medication reconciliation.
  • Monitor patient condition, identify changes in health status, and communicate significant findings to physicians and appropriate members of the care team.
  • Provide education and guidance to patients and caregivers regarding medications, disease management, treatment plans, safety, and overall health needs.
  • Directly and/or indirectly supervise care provided by Home Health Aides (HHAs) and Licensed Practical/Vocational Nurses (LPNs/LVNs).
  • Provide appropriate instruction, assign tasks, and conduct required supervisory visits in accordance with applicable state and federal regulations.
  • Accurately and thoroughly document patient visits in accordance with organizational policies and payer requirements.
  • Ensure documentation is completed and synchronized within required timelines.
  • Collaborate effectively with other members of the care team to promote continuity and quality of care.
  • Manage a patient caseload independently while maintaining clinical quality and regulatory compliance.
  • Travel to patients' homes throughout the assigned Boston-area coverage region.

Required Qualifications:

  • Active Massachusetts Registered Nurse (RN) License.
  • Strong clinical assessment and documentation skills.
  • Ability to work independently and manage patient care autonomously.
  • Ability to provide skilled nursing services in a home-based environment.

Preferred Qualifications:

  • Candidates with the following experience and attributes will be highly preferred:
  • Previous Home Health nursing experience.
  • Experience with OASIS assessments and documentation.
  • Nursing home experience, particularly with post-acute patients and assisted living patients.
  • Ability to work autonomously with minimal supervision.
  • Quick learner with the ability to onboard and become productive quickly.
  • Strong communication, organization, and care-coordination skills.
