Home Health RN – OASIS Experience Required | Vermont

Location: Newport, VT
Start Date: September 21, 2026
Duration: 25 weeks
Shift: Monday–Friday, 8:00 AM–5:00 PM
Weekend: Some weekend coverage required
Call: Limited/emergency basis
EMR: HealthWyse (Careficient)
Minimum Experience: 2 years
Certification: BLS required

Weekly Gross: $1940

Requirements:

• Active VT RN license
• Strong Home Health and Hospice experience
• OASIS experience is mandatory
• Experience conducting home visits and traveling between patients
• Ability to manage changing schedules and patient needs
• Valid driver’s license in good standing
• BLS/BCLS required
• Must be comfortable with outpatient/community-based care

Key Responsibilities:

• Provide skilled nursing care to patients in their homes.
• Complete OASIS assessments and required documentation.
• Develop and update individualized patient care plans.
• Coordinate care with physicians, therapists, caregivers, and other disciplines.
• Monitor patient conditions and respond appropriately to changes.
• Provide wound care and hospice-related nursing services as needed.
• Manage a flexible daily schedule while traveling between patient visits.
