🏥 UNIT / WORK AREA

RN will work on 2 Adult Mental Health Units (MHU):

• Medical Behavioral Health Unit
• Typical Adult Psychiatric Unit

The unit provides evaluation and treatment for patients of all ages diagnosed with acute psychiatric, behavioral, or mental health disorders.

👩‍⚕️ JOB RESPONSIBILITIES

• Coordinate the continuum of care from pre-admission through post-discharge
• Collaborate with patients, families, nurses, physicians, multidisciplinary teams, and healthcare benefactors
• Assist with development, implementation, revision, and reporting of the Case Management program
• Serve as a liaison between the patient/family and healthcare team
• Optimize patient outcomes
• Consult with the healthcare team regarding specific patient needs
• Help ensure patients and families move efficiently through the hospitalization process
• Provide excellent patient-centered care

✅ REQUIRED

Experience:
• 2 years of experience – Required
• Case Management skills – Required
• Epic experience – Required

License:
• Active RN license in the state of practice – Required

Certifications:
• BLS – Required
• TEAMS OR CPI – Required

⭐ PREFERRED

• BSN – Preferred
• Previous Care Management role experience – Preferred
• Behavioral Health experience – Preferred
• Case Management certification – Preferred
