🩺 JOB RESPONSIBILITIES

• Coordinate and deliver comprehensive end-of-life care to terminally ill patients
• Manage a caseload of hospice patients
• Conduct thorough patient assessments
• Develop individualized care plans
• Provide skilled nursing interventions
• Address physical, emotional, and psychosocial needs
• Manage patient symptoms
• Administer medications
• Educate patients and families
• Coordinate care with physicians and support staff
• Serve as the primary point of contact for patients and families
• Collaborate with the interdisciplinary hospice team
• Ensure communication and continuity of care
• Continuously evaluate the effectiveness of the care plan

🚨 REQUIRED

✅ Active RN License in the state of practice
✅  BLS certification from an approved vendor per AH policy
✅ Hospice Experience – REQUIRED
✅ Valid Driver's License
✅ Must be 21 years of age or older
✅ Current automobile insurance
✅ Reliable automobile

⭐ PREFERRED

• BSN – Preferred
• 5 years of Hospice Nursing Experience – Preferred
