RN Case Manager – Acute Care

💰 Pay Rate: $70/hr W2 | $2,800/week for 40 hrs

☀️ Shift: Days | 5x8-Hour Shifts

⏰ Hours: 8:00 AM – 4:30 PM | 40 hrs/week

📊 Case Load: Up to 25 patients
 

Requirements

• Active California RN License by start date
• Minimum 2 years of acute medical RN Case Manager/Care Manager experience
• Recent acute-care Case Management/Care Coordination experience within the last 3 months – HARD STOP
• Previous travel experience required – HARD STOP
• Epic experience required – HARD STOP
• Strong Care Coordination & Discharge Planning experience
• Must be able to independently arrange DME, SNF placement, and ambulance transportation
• Experience developing and coordinating Plans of Care
• Experience with HMOs/IPAs
• Knowledge of CMS, HIPAA, DRGs, Joint Commission standards, and other healthcare regulations
• Experience working with adult and geriatric populations
• Acute Medical/Surgical experience required; ED, ICU, and Surgical Services experience preferred

Key Responsibilities

• Assess patients and identify discharge and care coordination needs
• Develop and coordinate individualized plans of care
• Perform comprehensive discharge planning and transition-of-care coordination
• Independently coordinate DME, SNF placement, and ambulance transportation
• Organize and participate in interdisciplinary and family meetings
• Coordinate care for patients with complex psychosocial or discharge barriers
• Work with cases involving homelessness, mental health crises, substance-use treatment, protective services, advance directives, and POLST
• Maintain accurate and timely documentation in Epic
• Collaborate with physicians, nursing, social services, families, and external providers

Scheduling Details

• Department operates 7 days/week
• Variable 5-day workweek
• Days off may vary and are not guaranteed to be consecutive
• Floating to a nearby affiliated facility may be required
• Up to 7 days RTO
• 40 hours of orientation
