RN Case Manager – Eureka, CA
Location: Eureka, CA 95501
Start Date: October 5, 2026
Contract: 26 Weeks
Shift: Day | 5x8-Hour | 8:00 AM–4:30 PM
Position Highlights
- 2+ years of acute-care hospital Case Management experience
- Strong experience in Care Coordination & Discharge Planning
- Epic experience required
- Travel experience required — first-time travelers may be considered with strong experience
- Experience with HMOs, IPAs, Advance Directives & POLST
- Acute hospital experience across ED, ICU, Med/Surg, Pediatrics, and PCU/SDU/IMC/Observation
- Every other weekend required
- No call required
- Float between units based on department needs
- Locals accepted
- Free parking provided
