Description

This position is ideal for an experienced Acute Care RN Case Manager with strong expertise in care coordination, utilization review, medical necessity, and discharge planning.

Required Qualifications

Minimum 2 years of acute medical Care Manager experience
Strong experience performing Care Coordination and Discharge Planning
Acute hospital experience required
Case Management and Utilization Review experience required
Active California RN license required
Pending California license may be considered but must be active by the start date
