We are seeking a Travel Physical Therapist for a Home Health contract in The Villages, Florida. This opportunity offers the independence of home health while providing meaningful one-on-one patient care in the home setting.

Responsibilities

• Perform physical therapy evaluations and reassessments in the home setting.
• Develop and implement individualized patient care plans.
• Treat patients of varying ages and diagnoses to improve mobility, strength, safety, and independence.
• Provide gait training, balance training, therapeutic exercise, transfer training, and functional mobility interventions.
• Educate patients and caregivers on home exercise programs, safety strategies, and equipment use.
• Collaborate with physicians, nurses, case managers, and the interdisciplinary care team.
• Complete timely documentation and maintain home health compliance standards.
