Description

Responsibilities

Perform initial assessments of patients and families to determine home health needs
Complete comprehensive physical assessments and health histories
Re-evaluate patients' nursing needs regularly
Develop nursing diagnoses based on assessment findings
Initiate and revise individualized plans of care
Develop goals and provide therapeutic, preventive, and rehabilitative nursing interventions
Involve patients and families in the care-planning process
Administer medications and treatments as ordered
Provide patient/family education and health-care instructions
Identify and implement discharge planning needs
Act as Case Manager when assigned
Coordinate care for an assigned patient caseload
Prepare clinical notes and update physicians regarding patient status
Communicate changes in patient condition to physicians and obtain necessary orders
Coordinate required equipment, supplies, and services
Instruct, supervise, and evaluate home health aide services
Provide care across private homes, nursing homes, and other community locations

Requirements Active RN License BLS certification Home Health experience – Required Hospice experience – Required Current Driver’s License – Required Current Auto Insurance – Required Must be able to travel within the approximately 60-mile service area 50-mile radius rule: Candidate must reside outside 50 miles Cannot currently be or have been within the past year full-time, part-time, or PRN with a CommonSpirit/CHI/Dignity facility RTO must be submitted upfront; additional RTO will not be approved after submission BSN not required License to submit: Yes
