Description

Pride Health is actively hiring a Physical Therapist (PT) to join our client's Home Health Team.

 

Location: Arlington, TX 76015

Pay Rate: $65.00/hour (W-2)

Setting: Home Health

Schedule: Monday–Friday, 8:00 AM–4:30 PM

Hours: 40 hours/week

Productivity: 30 points/week

Mileage: Reimbursed between patient visits

Dress Code: Navy Blue Scrubs

Position Summary:

  • We are seeking a skilled Physical Therapist (PT) to provide quality home health services to patients in their homes.
  • The Physical Therapist will assess patients' functional abilities, develop individualized treatment plans, provide therapeutic interventions, and monitor patient progress to improve mobility, independence, and overall well-being.

Key Responsibilities:

  • Conduct comprehensive physical therapy assessments and evaluations.
  • Assess functional status, mobility, endurance, stamina, pain, and other medical conditions.
  • Develop and implement individualized treatment plans based on patient needs.
  • Provide therapeutic interventions to improve mobility, strength, balance, endurance, and functional independence.
  • Monitor and document patient progress and modify treatment plans as needed.
  • Educate patients and caregivers on appropriate exercises, safety, mobility, and home programs.
  • Coordinate care with physicians and the interdisciplinary healthcare team.
  • Maintain accurate and timely clinical documentation.
  • Coverage Areas: Arlington, Burleson, Mansfield, DeSoto, Waxahachie, Cleburne, and Red Oak, TX.

Required Qualifications:

  • Current Physical Therapist (PT) certification/license.
  • Physical Therapy experience, preferably in a home health setting.
  • Strong clinical assessment and patient-care skills.
  • Ability to work independently and manage a daily patient caseload.

Preferred Skills:

  • Excellent communication and interpersonal skills.
  • Strong time-management and organizational abilities.
  • HCHB proficiency preferred.
  • Experience providing care in patients' homes is a plus.
