Job: RN, Care Manager
Dates: 10/12/2026 – 01/10/2027
Length: 13 weeks
Schedule: Days, 8:00 AM–4:30 PM, Tuesday–Saturday
Hours: 40 hrs/week (5×8)
On-call: No
Float: No
Main requirements
- RN CM certification — REQUIRED
- Inpatient Case Management experience — REQUIRED
- Heavy caseload experience
- Experience handling fast-paced/high-volume CM work
- Strike-force experience with Kaiser Permanente (KP) or HCA California preferred
- Must be comfortable with minimal training and managing a heavy caseload
Main responsibilities
- Coordinate the continuum of care from pre-admission through post-discharge.
- Develop, implement, revise, and report on case-management plans.
- Coordinate with patients, families, physicians, nurses, multidisciplinary teams, and payers/benefactors.
- Serve as a consultant to the healthcare team.
- Help patients and families move efficiently through hospitalization.
- Optimize patient outcomes and coordinate discharge planning.
Candidate must have
Must-have:
- Active RN license appropriate for CA assignment
- RN Case Manager certification
- Inpatient CM experience
- Available for Tuesday–Saturday, 8:00 AM–4:30 PM
- Able to work 40 hours/week
- Comfortable with a heavy caseload
Preferred: KP/HCA California strike-force experience.
