Description
The RN Case Manager provides excellent patient care by collaborating in the development, implementation, revision, and reporting of the case management program. Acts as a liaison between the patient, family, nurses, physicians, multidisciplinary care team, and healthcare payer to help optimize patient outcomes.
The RN Case Manager serves as a consultant and resource to the healthcare team regarding patient-specific needs and coordinates the continuum of care for an assigned population from pre-admission through post-discharge. Ensures that patients and families progress efficiently through their hospitalization while supporting appropriate care coordination and discharge planning.
Requirements
- Position: RN Case Manager
- Shift: Day Shift, 8:00 AM–4:30 PM
- Schedule: Sunday–Thursday or Tuesday–Saturday, depending on assignment
- Hours: 40 hours/week
- No on-call requirement
- RN Case Management (RN CM) certification required
- Inpatient Case Management (IP CM) experience required
- Candidates with strike-force experience are preferred, particularly those familiar with Kaiser Permanente (KP) or HCA California
- Experience working with heavy caseloads is strongly preferred
- Candidates should be able to adapt quickly and require minimal training
