Position: Registered Nurse – Case Manager
Location: Eureka, CA 95501
Start Date: 10/05/2026
Contract: 13 Weeks
Shift: Days | 5×8 | 8:00 AM–4:30 PM
Weekend: Every Other Weekend
Call: Not Required
Charting: Epic
Patient Ratio: 1:20
Case Load: Up to 25

REQUIRED:

Active RN license
Minimum 2 years of acute-care hospital Case Management experience
Acute-care Case Management experience must be current within the last year
Care coordination and discharge planning experience
Epic experience
Experience with HMOs, IPAs, etc.
Experience with Advance Directives, POLST, and Protective Services processes
Case Management / Utilization Review experience
Admission criteria, benefits eligibility, needs assessment and DME coordination
CMS, HIPAA, Joint Commission/Core Measures/National Patient Safety Goals knowledge
Experience across acute-care settings such as ED, ICU, Med/Surg, Pediatrics, and PCU/IMC/Obs preferred
