REQUIRED:

Active California RN License — Pending accepted if licensed by start
2 years acute medical Care Manager experience
Recent Care Coordination experience within the last 3 months
Travel RN experience – HARDSTOP
Epic experience – HARDSTOP
Acute hospital Case Management experience
Care Coordination & Discharge Planning
DME, SNF & ambulance transport arrangements
Experience with HMOs/IPAs
Family meetings & treatment planning
Strong psychosocial assessment and resource coordination skills
