Requirements
Bachelor's Degree in Nursing (BSN): Preferred
Registered Nurse (RN) licensure in the state of practice: Required
Basic Life Support (BLS) certification from approved vendor: Required
Experience in a care management role: Preferred
Case management certification: Preferred
At least two years recent experience in field: Required
Responsibilities
Lead the coordination of patient care with the healthcare team, ensuring the appropriateness and timeliness of care.
Monitor patient transitions through the care continuum from pre-admission to post-discharge.
Act as a liaison among patients, families, and healthcare providers, optimizing patient outcomes.
Collaborate on complex cases, including frequent emergency department visitors and specific patient populations.
Coordinate transfers to other facilities, ensuring continuity of care and required patient information.
Collect and manage data to support regulatory and accreditation compliance.
