Hospice RN – Home Health | Weekend Program

📍 Location: Green Bay & Oshkosh, WI markets
🕐 Shift: 8:00 AM–8:00 PM, Friday–Sunday only
📅 Contract: 13 weeks
📅 Weekend: Every weekend – Fri/Sat/Sun schedule
🚗 Setting: Home Hospice – visits patients in their homes
💻 EMR: Epic (Rover) / Remote Client
👕 Scrubs: Navy Blue

Job Details

Home Hospice RN providing care in patients' homes.
Patient population includes failure to thrive, advanced cancers, end-stage COPD, terminal illness, and end-stage disease processes.
Perform focused assessments, symptom management, wound care, Foley exchanges, feeding tube troubleshooting, post-fall assessments, and death visits.
May perform PRN after-hours visits for symptoms, falls, death visits, etc.
RN caseload: 0–6 depending on call-ins.
May occasionally assist slightly outside assigned territory if closer to a patient; generally no more than 1 hour from home territory.
Epic Rover/Remote Client
Orientation: 3 shifts (1 week) with an SCT nurse
Lead RN available 24/7 for support

Requirements

2+ years Home Health or Home Hospice experience
First-time travelers accepted if they have home-setting case management experience
BLS
Active Wisconsin RN license
Active Driver’s License
Own vehicle + auto insurance
