Patient Ratio: 1:25
Weekend: Rotation required

Requirements

• 2 years of Acute Care Case Management/Discharge Planning experience required
• Seasoned travel RN experience required
• Must have hospital experience; behavioral health experience alone does not qualify
• Strong experience in Case Management and Utilization Review
• Experience with patient assessment, care coordination, and discharge planning
• Experience with admission criteria, benefits eligibility, and prior authorizations
• Knowledge of CMS, DRGs, Department of Health, Joint Commission/Core Measures
• Acute hospital experience with ED, ICU, Med-Surg, and Surgical Services populations preferred
• Must independently arrange DME, SNF placement, and ambulance transportation
• Experience working with HMOs/IPAs
• Strong communication skills and ability to participate in family meetings and treatment planning
• Computerized documentation experience required

Key Responsibilities

• Assess patients and develop appropriate plans of care
• Coordinate services with physicians, nursing, social services, and other interdisciplinary teams
• Perform utilization review and ensure appropriate level of care
• Coordinate discharge planning and post-acute services
• Arrange DME, SNF placement, and transportation
• Assist with family meetings and treatment-plan development
• Support patients with complex social and healthcare needs
• Maintain accurate and timely documentation

Patient Population: Neonates, Adolescents, Adults, and Geriatrics
Orientation: 40 hours
Interdisciplinary Support: Physical Therapy and Social Services
