Requirements

Minimum 2 years of recent acute-care Medical Case Management experience
Recent Care Coordination experience required, ideally within the last 3 months
Travel experience required – first-time travelers are not accepted
Active/valid California RN license required by start date
Epic experience required
Experience with assessment, care coordination, and discharge planning
Ability to arrange DME, SNF placement, and ambulance transportation independently
Experience with HMOs, IPAs, and family meetings preferred
Strong knowledge of discharge planning and patient resource coordination
