Schedule: Day Shift | Monday–Friday
Hours: 40 hours/week
Work Setting: Onsite – Portland, OR
RN License: Required
Case Management Certification: Preferred

Position Overview

We are seeking an experienced Registered Nurse (RN) with strong case management and healthcare experience to support patients/members throughout their healthcare journey.

The ideal candidate will have 5–10 years of experience across acute care and ancillary healthcare settings, with experience in multiple clinical modalities. Experience working within a health plan, particularly in utilization management and case management, is preferred.

Strong communication and patient/member coaching skills are essential for this role.

Key Responsibilities

• Support and guide patients/members throughout their healthcare journey
• Apply clinical knowledge to case management and utilization management activities
• Communicate effectively with patients, members, providers, and healthcare teams
• Help patients/members understand and navigate their healthcare options and services
• Collaborate with multidisciplinary teams to support appropriate and coordinated care
• Utilize experience across acute care and ancillary healthcare settings

Requirements

Education & Experience

• Bachelor’s Degree in Nursing (BSN): Preferred
• 5–10 years of acute and ancillary healthcare experience: Preferred
• Experience across multiple healthcare modalities
• Health plan experience with utilization management and/or case management: Preferred
• Strong communication and patient/member coaching skills

Licensure & Certifications

• Active RN License in the state of practice: Required
• Certified Case Manager (CCM): Preferred
