Case Manager - LVN - Days
Start Dates: ASAP
Orientation Dates: ASAP (Paid Orientation—3 days)
- 13-week contract
- 40 hours
- 5 x 8 ’s
- Shift: Days
- 9am-5:30pm with alternating weekends
Experience:
- 1 year of Case Manager experience and 2 years of LVN experience
- Primary focus: Patient placement/discharge planning
- Required: Strong working knowledge of Skilled Nursing Facilities (SNFs) and common post-acute placement options.
- Required: Proven expertise in complex discharge dispositions and care transitions.
- Required: Able to consistently use the appropriate terminology associated with discharge planning
- Certs: BLS, LA County Fire Card
License: Active CA LVN License
