Description

Experience:

1 year of Case Manager experience and 2 years of LVN experience
Primary focus: Patient placement/discharge planning — the majority of the job is coordinating where patients go next and ensuring a smooth transition.
Required: Strong working knowledge of Skilled Nursing Facilities (SNFs) and common post-acute placement options.
Insurance awareness: Understanding how insurance impacts placement decisions (used by the Case Manager to secure/approve appropriate placements).
Placement types to know:
- Acute rehab
- Board & care (boarding care)
- SNF/SNF placement

Orientation: The LVNs will complete a structured 5-day training program at KP LAMC, which includes 2 days of required online education followed by 3 days of orientation with a preceptor to support hands-on learning and integration into the team.
