Description

Job Title: Home Health/Hospice RN
Facility Type/Setting: Home Health & Hospice – Outpatient Care
Shift & Schedule: Fri, Sat, Sun, and Mon; 8:00 AM–6:00 PM
Contract Length: 24 weeks
Guaranteed Hours: 40 hours/week

Call / Weekend / Holiday Requirements: Some call may be scheduled, but only on an emergency basis; Friday–Monday schedule includes weekends

Experience Required / Preferred: OASIS experience required; Home Health and Hospice experience required; experience traveling from patient to patient for home visits

Requirements:

OASIS (Outcome and Assessment Information Set) experience
Home Health and Hospice experience
Experience conducting patient-to-patient home visits
Ability to manage and adjust to schedule changes
Valid driver's license in good standing
BLS required

Responsibilities:

Provide outpatient care through home visits
Travel from patient to patient for home visits
Manage and adjust to changes in the schedule
Provide care in the Home Health and Hospice setting
