We are seeking an experienced RN Care Coordinator to coordinate the care and services of assigned patients throughout their episode of illness or treatment. The Care Coordinator will work closely with patients, families, physicians, the multidisciplinary care team, and payors to ensure appropriate, timely, and cost-effective care.

This role combines clinical assessment, case management, utilization management, care coordination, discharge planning, and patient advocacy.

Key Responsibilities

• Coordinate care and services for assigned patients from admission through discharge or transfer.
• Perform clinical assessments using the nursing process, including assessment, planning, intervention, and evaluation.
• Collaborate with patients, families, physicians, nurses, multidisciplinary teams, and payors.
• Monitor patient progress throughout the episode of care.
• Evaluate appropriate utilization of healthcare resources.
• Review admission, discharge, and observation/1-day stay criteria.
• Evaluate quality and utilization cases.
• Review 30-day readmissions and identify potential care coordination opportunities.
• Perform Medicare and diagnosis-related criteria reviews.
• Participate in ongoing outlier reviews.
• Coordinate appropriate services and resources to support timely discharge.
• Identify barriers to discharge and work with the care team to resolve them.
• Advocate for patients and families throughout the entire episode of care.
• Serve as a resource for patients and families and facilitate communication among providers.
• Monitor services being provided to ensure appropriate care and resource utilization.
• Remain involved until the patient is discharged, transferred to an acute-care setting, or transitioned to an appropriate alternate level of care.
• Initiate appropriate clinical pathways based on the patient's admission diagnosis.
• Serve as a clinical resource for utilization management activities.
• May be assigned to the Emergency Department to assist with care coordination and utilization management.
• Promote efficient and cost-effective care while maintaining quality and patient-centered outcomes.

Minimum Requirements

• Minimum 1 year of recent Case Management experience in an acute-care setting.
• Current California RN License.
• Current CPR recognition/certification.
• Strong clinical assessment, communication, and care coordination skills.
