Description
The Home Health/Hospice RN provides skilled nursing care to patients in their homes. The RN is responsible for assessing patients, developing and implementing individualized care plans, monitoring patient progress, coordinating care with the healthcare team, and completing accurate clinical documentation.
Key Responsibilities:
- Travel to patients' homes and provide skilled nursing care according to the patient's individualized plan of care.
- Perform comprehensive patient assessments and reassessments.
- Complete accurate OASIS (Outcome and Assessment Information Set) assessments and documentation.
- Provide home health and hospice nursing services based on patient needs.
- Monitor patients' conditions and identify changes that require intervention or escalation.
- Administer medications and treatments as ordered.
- Provide wound care and other skilled nursing procedures as appropriate.
- Educate patients and caregivers regarding medications, disease management, treatments, safety, and care plans.
- Coordinate care with physicians, therapists, hospice/home health staff, and other members of the interdisciplinary team.
- Communicate significant changes in patient condition to the appropriate provider or care team.
- Maintain timely and accurate clinical documentation in HealthWyse (Careficient).
- Manage and adjust to changes in the daily visit schedule while maintaining quality patient care.
- Maintain patient confidentiality and follow applicable clinical and organizational policies.
- Participate in emergency/on-call coverage when scheduled.
Required Qualifications:
- Previous Home Health and Hospice experience required.
- Strong OASIS experience required.
- Experience providing care through in-home patient visits.
- Ability to independently travel from patient to patient.
- Ability to adapt to changing schedules and patient needs.
- Valid driver's license in good standing.
- Current BLS certification.
- Strong assessment, communication, organization, and documentation skills.

Additional Requirement:
The facility will conduct its own background check as part of the hiring process.
