Description

hift: Days, 5x8s — 8:00 AM–4:30 PM
• Experience: 1 year Skilled Home Health PT experience required
• First-Time Travelers: Accepted
• Travel Experience: Not required
• Certification: BLS required
• License: Oregon/Compact license; pending license accepted
• Home Health: Required
• Caseload: 1:1 visits; up to 6 patients/day
• Productivity: 30 points standard
• Transportation: Must have reliable personal transportation
• Mileage: IRS standard mileage reimbursement
• Service Area: Seaside and surrounding areas; occasional driving up to approximately 60 miles
• Charting: Epic Remote Client, Rover & Oasis
• Oasis: Required
• Weekend: No
• Call: No
• Float: No
• Block Scheduling: No
• RTO: Must be submitted upfront and confirmed with hiring manager
• Guaranteed Hours: Facility may cancel up to 1 shift per pay period
• Local Candidates: Accepted
• COVID Vaccine: Primary series + 1 booster required
• Flu Vaccine: Unknown
• Holiday: Must commit to working 3 of 5 holidays — Thanksgiving, Christmas Eve, Christmas Day, NYE or NYD
• Providence Submission: Can only submit to 1 Providence facility at a time
• References: 2 references covering 1 year within the last 3 years
