ALL APPLICANTS MUST HAVE Physical Therapist Professional license in Massachusetts. 1 Year Home Care experience required. Weekend rotations are once every 6 weeks. Ability to travel to cases as assigned. Job Summary: Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations or disabilities resulting from injury, disease, or other causes. Patients may range in age from birth to over 100 years. Collaborate with patient, family, physician, and other allied health professionals to identify patient needs and coordinate services to meet those needs. Utilizing thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice of MGH Home Care.ALL APPLICANTS MUST HAVE 1 year Certified Home Care experience. 1 year Case Management Experience.Please note: Experience in hospice, private care, or working as a Case Manager on its own does not meet the qualifications for Certified Home Health Care Management experience. Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement 
Unit Description: Back Bay, Beacon Hill, Boston, Cambridge, Charlestown, Chinatown, Fenway, Financial District, Kenmore, North End, Prudential, South Boston, South End, West EndStart date based on credentialing clearance. If not cleared by posted start date-a rescheduled start date will be necessary
