Looking for your next rewarding travel PT assignment? Ardor Health Solutions is seeking a Travel Physical Therapist for a Skilled Nursing Facility (SNF) opportunity in Peoria, Illinois! Help patients regain their strength, mobility, and independence while enjoying a welcoming Midwestern city along the Illinois River.
What You'll Do
- Provide skilled physical therapy services to residents in a SNF setting.
- Perform comprehensive evaluations and develop individualized treatment plans.
- Help patients improve strength, balance, mobility, and functional independence.
- Treat patients recovering from surgery, illness, injury, or hospitalization.
- Collaborate with nursing staff and the interdisciplinary rehabilitation team.
- Assist with discharge planning and patient education.
- Complete accurate and timely clinical documentation.

Requirements
- Active Illinois Physical Therapist license or eligibility to obtain.
- SNF/LTC experience preferred.
- Current BLS certification.
- Strong clinical reasoning, communication, and patient care skills.
- Ability to work independently and collaborate with an interdisciplinary team.
