What You'll Do

• Provide skilled physical therapy services to residents in a SNF setting.
• Perform comprehensive evaluations and develop individualized treatment plans.
• Help patients improve strength, balance, mobility, and functional independence.
• Treat patients recovering from surgery, illness, injury, or hospitalization.
• Collaborate with nursing staff and the interdisciplinary rehabilitation team.
• Assist with discharge planning and patient education.
• Complete accurate and timely clinical documentation.

Requirements

• Active South Dakota Physical Therapist license or eligibility to obtain.
• SNF/LTC experience preferred.
• Current BLS certification.
• Strong clinical reasoning, communication, and patient care skills.
• Ability to work independently and collaborate with an interdisciplinary team.
