Position Title: Registered Nurse (RN) – Home Health
Location: Needham Heights, MA 02494
Coverage Area: Boston and surrounding regions including, but not limited to, South Boston, Dorchester, Back Bay, Roxbury, Beacon Hill, and South End
Pay Rate: Up to $58.00/hour (W2)
Hours Guaranteed: 32 Hours per Week
Schedule: Monday through Friday, 8:00 AM – 4:30 PM
Weekend/On-Call Requirement: Yes
Mileage Reimbursement: Yes (between patient visits, from first patient to last patient)

Position Summary

We are seeking a compassionate and experienced Registered Nurse (RN) to provide high-quality home health care services throughout the Boston metropolitan area. The RN will be responsible for delivering patient-centered care, performing comprehensive assessments, developing and managing individualized care plans, and coordinating services with physicians and interdisciplinary team members to ensure optimal patient outcomes.

Key Responsibilities

• Initiate, develop, implement, and revise individualized patient care plans in collaboration with physicians and other healthcare professionals.
• Perform comprehensive patient assessments, including OASIS documentation and medication reconciliation.
• Provide skilled nursing care in accordance with physician orders, agency policies, infection control standards, and home health regulations.
• Supervise and support Home Health Aides (HHAs) and Licensed Practical/Vocational Nurses (LPNs/LVNs), including conducting required supervisory visits and providing clinical instruction.
• Coordinate care with members of the interdisciplinary healthcare team to ensure continuity and quality of care.
• Accurately document all patient encounters, assessments, interventions, and outcomes in a timely manner according to agency, regulatory, and payer requirements.
• Maintain compliance with state, federal, and accreditation standards.
• Educate patients and caregivers regarding disease management, medications, treatments, and safety measures.
• Manage a caseload independently while prioritizing patient needs and maintaining productivity standards.

Required Qualifications

• Active Massachusetts Registered Nurse (RN) License.
• Graduate of an accredited nursing program.
• Current BLS certification.
• Minimum of one year of nursing experience preferred.
• Strong assessment, clinical judgment, and care coordination skills.
• Ability to travel throughout assigned coverage areas.
• Valid driver's license and reliable transportation.

Preferred Qualifications

• Previous Home Health experience.
• Experience working with post-acute care, skilled nursing facility (SNF) patients, and assisted living populations.
• Familiarity with OASIS assessments and documentation standards.
• Ability to work independently with minimal supervision.
• Quick learner with the ability to onboard and adapt rapidly to new systems and processes.
• Excellent communication, time management, and organizational skills.
