UNIT: LONG TERM CARE

Required Certs:  WI RN, AHA BLS

Minimum Experience: 2+ years LTC experience REQUIRED & 1 completed contract (preferred) Travel experience strongly preferred.  Long term care experience highly preferred; Will accept acute care experience in lieu of LTC if an experienced RN

Charting Experience: EPIC strongly preferred

Required Skillset: Worked in LTC/Assisted Living and Rehab 
Skilled Nursing Facility

Patient Ratio: 1:10 in Rehab & 1:24 LTC/Assisted Living (with CNA assistance of 1:10 ratio)

**any missed shift must be made up within 2 weeks of the absence
Weekends: Every other

Holidays: As needed

Float Requirements: Throughout facility within skillset, To all ThedaCare facilities within a 60 mile radius

On Call/Call Back Requirements:  None
Scrub Colors: black (no hats or hoodies. Scrub jackets okay)

8hr unbillable orientation

GSA hours are not guaranteed the week of Orientation. Traveler is guaranteed set hours per week, except during holidays where unit is closed.
