🌴 Travel MSW – Acute Care Social Worker | Santa Monica, CA

✨ Position Highlights

• Join an acute-care inpatient medical social work team in a highly desirable Southern California location.
• Work alongside an interdisciplinary team supporting patients through assessment, care coordination, crisis intervention, and discharge planning.
• Excellent opportunity for an experienced MSW looking to expand their acute hospital and care-management experience.
• Epic experience required, allowing you to step into an established electronic health record environment.
• No floating and no call requirement.
• Every-other-weekend rotation: 2 weekends in a 4-week period.
• 40 hours of orientation provided.
• First-time travelers may be considered, although prior travel experience is preferred. 

🏥 Role & Responsibilities

The Social Worker will support patients and families throughout the inpatient care journey, including:

• Conduct biopsychosocial assessments and initial evaluations.
• Assess patient and caregiver needs and develop appropriate plans of care.
• Provide care coordination and discharge planning for acute-care patients.
• Coordinate home health, DME, rehabilitation, long-term care, and community resources.
• Collaborate with physicians, nurses, case managers, therapists, and other interdisciplinary team members.
• Participate in family meetings, care conferences, and treatment-plan development.  
• Provide crisis intervention and support for patients experiencing mental health, safety, or psychosocial concerns.
• Assist patients and families with advance directives, POLST, guardianship, Power of Attorney, and protective-service processes.
• Support patients experiencing homelessness, substance-use concerns, mental health crises, or other complex social needs.
• Coordinate with HMOs, IPAs, and other insurance/community resources as appropriate.
• Maintain timely and accurate documentation in Epic.
• Facilitate safe and appropriate transitions of care. 

🧠 Required Experience & Skills

• MSW required
• 2+ years of acute medical hospital inpatient Social Work experience
• Experience performing both care coordination and social work functions
• Epic experience required
• Travel experience preferred; first-time travelers may be considered
• Strong experience with:
  • Biopsychosocial assessments
  • Care coordination
  • Discharge planning
  • Crisis intervention
  • Community resources
  • Patient/family counseling
  • Protective services
  • Advance directives/POLST
  • DME and post-acute placement
  • HMOs/insurance coordination 

⭐ Preferred Qualifications

• LCSW
• Experience with 5150/involuntary holds
• Experience with InterQual criteria
• Knowledge of drug/alcohol treatment resources
• Adoption and foster-care experience
• Experience with psychiatric or crisis patients
• ED Social Work experience
• Familiarity with L&D/Mother-Baby or NICU populations

• Average daily census: approximately 22–25 patients

• 13-week travel contract
• Day shift
• 3x12-hour schedule

• RTO allowance: up to 7 days
• COVID vaccination: not required per facility guideline
