Travel Occupational Therapist – SNF/Rehab

Hale Makua Wailuku | Wailuku, Maui, HI

Ardor Health Solutions is seeking an Occupational Therapist for a travel SNF/Rehab contract at Hale Makua Wailuku in Wailuku, Maui, Hawaii.

Facility & Position

Hale Makua Wailuku is a skilled nursing facility offering both short-term rehabilitation and long-term nursing care. Its multidisciplinary rehabilitation program includes occupational, physical and speech therapy and treats patients recovering from conditions including fractures, joint replacements, strokes and acute illnesses.

The Occupational Therapist will work primarily with an adult and geriatric population, helping patients regain the skills needed for greater independence and a safe transition to their next level of care.

Responsibilities may include:

OT evaluations and reassessments
ADL and self-care retraining
Upper-extremity strengthening and functional activities
Transfer and functional mobility training
Adaptive equipment training
Cognitive and safety interventions
Patient and caregiver education
Individualized treatment planning
Collaboration with PT, SLP, nursing and the interdisciplinary team
Discharge planning and preparation for a safe return home
