Position Overview

The Pharmacist will provide inpatient pharmacy services while supporting safe and effective medication management. The role requires strong clinical pharmacy experience, particularly with pharmacokinetic dosing and anticoagulation management.

Key Responsibilities

• Provide professional pharmacy services in an inpatient hospital setting
• Review and manage medication therapy
• Perform pharmacokinetic dosing, particularly vancomycin dosing
• Manage warfarin therapy
• Support safe and effective medication use
• Collaborate with healthcare providers and the pharmacy team
• Perform pharmacy duties according to hospital policies and professional standards

Required Qualifications

• Active Ohio Pharmacist License in good standing – Required
• Minimum 1 year of inpatient hospital pharmacy experience – Required
• Experience with pharmacokinetic dosing, specifically vancomycin – Required
• Experience with warfarin management – Required
• Must be willing to work evening and weekend shifts

Preferred Qualifications

• Epic Willow Inpatient experience – Preferred, but not required

Additional Information

• The facility reserves the right to cancel up to 36 hours during the 13-week contract based on staffing needs.
• Weekend scheduling may vary depending on facility staffing.
