Ardor Health Solutions is looking for a Home Health - LPN to join our travel team near Fort Wayne, IN! This is a full time travel contract position.
Requirements include, but are not limited to:
- Active yes LPN Productivity Requirement: 30 points per week license or Compact
- 1+ year of recent Home Health - LPN experience
- HCHB experience required
- Shift: Mon-Fri 0800-1700 Are 40 hours guaranteed: yes
- Coverage area: Allen, Whitley, Kosciusko, Huntington, Wabash Required Skills: must be comfortable with basic nursing skills such as venipuncture, wound care, disease management teaching. Preferred experience with HCHB/Pointcare in Home Health. The Licensed Practical Nurse is a licensed professional who provides direct and indirect patient care services to patients in their home environment or alternative setting. The LPN collaborates with the Registered Nurse to provide nursing treatments to patients according to the plan of care established. The LPN provides technical nursing interventions including infusion, wound care, phlebotomy, ostomy, catheter care, and other skilled interventions as ordered by the physician and within their Scope of Practice. The LPN instructs, motivates, and assists patient and/or caregivers in learning and improving ability to live independently at home.
- Call requirement: On Call Schedule: no
- Productivity expectations: 30 points
- Mileage reimbursement: $0.47 per mile
