Looking for your next opportunity in Hospital Case Management? This 13-week contract in Billings, MT is a great opportunity for an experienced RN Case Manager to join a collaborative care management team and play a key role in coordinating quality patient care.

You'll work closely with patients, families, physicians, nurses, and multidisciplinary teams to develop effective care plans, coordinate services, and support safe and appropriate discharge planning.

🩺 Key Responsibilities

• Coordinate care and services for patients requiring Case Management support.
• Conduct comprehensive assessments of patients' medical, functional, and social needs.
• Develop individualized care plans in collaboration with patients, families, and the healthcare team.
• Coordinate and facilitate discharge planning.
• Assess patients' prior level of functioning and available community resources/support systems.
• Help determine appropriate care settings and continued service needs.
• Collaborate with physicians and multidisciplinary healthcare teams.
• Identify admission and continued-stay needs.
• Support appropriate utilization of healthcare resources and strategies to reduce unnecessary length of stay.
• Participate in rounds, case conferences, and multidisciplinary meetings.
• Advocate for patient needs and rights.
• Track outcomes, collect data, and monitor care management effectiveness.
• Provide education regarding case management and discharge planning processes.
• Connect patients and families with appropriate community resources.

✅ Requirements

• Active RN license in Montana or a Compact RN license
• BSN required for this opportunity
• Minimum 3 years of RN experience
• Acute care experience required
• Hospital-based Care Management experience required
• Knowledge of medical terminology
• Strong computer skills, including data entry and familiarity with Excel and PowerPoint
• BLS/CPR required

⭐ Preferred Qualifications

• 5+ years of nursing experience
• ACM or CCM certification is preferred but not required
• Previous experience with hospital case management, utilization management, and discharge planning
