Travel Physical Therapist – SNF/Rehab
Ginger Cove | Annapolis, MD
Ardor Health Solutions is seeking a Physical Therapist for a travel SNF/Rehab contract at Ginger Cove in Annapolis, Maryland.
Position Overview
This position works primarily with an adult and geriatric rehabilitation population, helping patients improve mobility, strength, balance and functional independence following hospitalization, surgery, illness or injury.
The Physical Therapist will evaluate patients, develop individualized plans of care and work closely with the interdisciplinary team to help patients achieve their highest possible level of function and safely transition to their next level of care.
Responsibilities may include:
- PT evaluations and reassessments
- Development and progression of individualized treatment plans
- Therapeutic exercise and strengthening
- Gait and balance training
- Transfer and functional mobility training
- Fall prevention and safety education
- Assistive device recommendations and training
- Patient and caregiver education
- Collaboration with OT, SLP, nursing and the interdisciplinary rehab team
- Discharge planning
