Travel Physical Therapist – Home Health
Lewistown, MT
Ardor Health Solutions is seeking a Physical Therapist for a travel Home Health contract in Lewistown, Montana.
Position Overview
This position provides 1:1 physical therapy in patients’ homes, giving the therapist greater autonomy and the opportunity to provide individualized care without the pace and competing demands of a traditional facility setting.
The PT will evaluate patients in their home environment, develop individualized plans of care and help patients improve mobility, strength, balance and safety while working toward greater independence.
Responsibilities may include:
- Home Health PT evaluations and reassessments
- Development and progression of individualized plans of care
- Therapeutic exercise and strengthening
- Gait and balance training
- Transfer and functional mobility training
- Fall-risk assessment and prevention
- Home safety assessment
- Assistive device recommendations and training
- Patient and caregiver education
- Coordination with nursing, OT, SLP and other members of the Home Health team
- Documentation and communication of patient progress
