Travel Physical Therapist – Home Health

Fall River Health Services | Hot Springs, SD

Ardor Health Solutions is seeking a Physical Therapist for a travel Home Health contract with Fall River Health Services in Hot Springs, South Dakota.

Position Overview

This position provides 1:1 physical therapy in patients’ homes, allowing the PT to deliver individualized care while working with a high level of autonomy.

The therapist will evaluate patients in their home environment, develop individualized plans of care and help patients improve mobility, strength, balance and safety while working toward greater independence.

Responsibilities may include:

Home Health PT evaluations and reassessments
Development and progression of individualized plans of care
Therapeutic exercise and strengthening
Gait and balance training
Transfer and functional mobility training
Fall-risk assessment and prevention
Home safety assessments
Assistive device recommendations and training
Patient and caregiver education
Collaboration with nursing and other members of the interdisciplinary team
Documentation of patient progress and outcomes
