Travel Physical Therapist (PT) – SNF & Home Health

Setting: SNF & Home Health – 50/50 split
Shift: 8-hour Days

Guaranteed Hours: 40 hours/week for 8–10 hour shifts
Charting: Homecare Homebase (HCHB)
Required Certification: BLS

Job Description

Seeking an experienced Physical Therapist (PT) for a combined Skilled Nursing Facility (SNF) and Home Health assignment. The workload is approximately 50% SNF and 50% Home Health, but this may fluctuate based on facility needs.

The clinician may occasionally provide coverage in Hastings or Grand Island for either SNF or Home Health services.

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Provide physical therapy treatments based on patient needs
• Perform mobility, strength, balance, and functional assessments
• Help patients improve mobility and independence
• Document evaluations, treatments, and progress in HCHB
• Coordinate care with nurses, physicians, and other healthcare professionals
• Provide patient/family education and home exercise programs
• Follow facility and home-health safety and documentation standards

Requirements

• Active PT License – required at submission
• BLS Certification
• SNF and/or Home Health experience preferred
• Ability to work across both SNF and Home Health settings
• Willingness to occasionally cover Hastings or Grand Island
• Strong documentation and communication skills

Important Assignment Details

• 80-mile local rule: Local candidates within 80 miles are subject to local placement rates
• Local adjusted bill rate: -$8
• Local adjusted OT bill rate: -$8
• No on-call or call-back available
• No charge differential at Good Samaritan facilities
• Work week: Sunday–Saturday
• Cancellation policy: 2 weeks' notice
• A $500 fee may be billed if the clinician backs out or quits with less than 14 days' notice
• If GSS housing is used, local rates apply
